All students attending our West Palm Beach campus are required to complete a Health Information Form. All first-year, full-time undergraduate and international students must have a physical examination and submit a Physical Form, completed by a medical professional within the past 12 months. Students must also submit proof of required immunizations which are stated on the Immunization Form. All international students (or U.S. Citizens born outside of the U.S.) must submit a TB Risk Screening Form in addition to the other forms. All forms must be submitted to the Health and Wellness Center prior to August 1 (for students starting in the Spring semester the forms should be submitted prior to December 1). All full-time day students (undergraduate, international, and pharmacy) are required to provide proof of health insurance at the start of each academic year. Students will be automatically billed for the student health insurance and receive a credit when their insurance waiver is approved.
