Fall 2013: The verification worksheet for fall 2013 (2013-14 award year) is now an online form. Students must login to myPBA to complete the form. From the home tab, click on "View Financial Aid Information" and then on "My Required Documents". Finally, click on the "Complete it Now" button.
Summer 2012/13: The verification worksheet for summer 2012/13 students (2012-13 award year) is in PDF format. To download a copy login to myPBA. From the home tab, click on "View Financial Aid Information" and then on "My Required Documents". Finally, click on the Verification Worksheet link to access the form.

IRS Data Retrieval / IRS Tax Return Transcript
If you are selected for verification we cannot accept a copy of your Federal tax return. Instead, you must either use the IRS Data Retrieval Tool on your FAFSA or submit a copy of your IRS Tax Transcript to our office. If you have already submitted your FAFSA, you can make a correction to it by using the IRS Data Retrieval Tool.
