- Academic/Character Recommendation Waiver Form - Applicants must fill out this online form before any recommendation is submitted. You will be asked to create an online account before completing the form.
- Academic Recommendation Form - This is an online form that must be filled out by a teacher or guidance counselor of the applicant. The Academic reference should create a unique login to complete the form. Please do not use a login given to you by a student.
- Admission Application - Please submit with $30 application fee and a personal essay about your present and future goals. Or apply online.
- Admission Application for Evening Programs or apply online- Please submit with $35 application fee.
- Admission Application for Graduate Programs or apply online - Please submit with $45 application fee.
- Application for Readmission - All formerly enrolled students who wish to come back to PBA must complete the application for readmission form. This is the first step in the re-admission process.
- Character Recommendation Form - This is an online form that must be filled out by an adult, other than a family member, who can give an accurate description of the prospective student's character. The Character reference should create a unique login to complete the form. Please do not use a login given to you by a student.
