- Character Recommendation Form - This is an online form that should be completed by a non-family member such as a pastor, mentor or teacher who can attest to the applicant's character.
- Admission Application - Please submit with $35 application fee and a personal essay about your present and future goals. Or apply online.
- Admission Application for Evening Programs or apply online- Please submit with $35 application fee.
- Admission Application for Graduate Programs or apply online - Please submit with $45 application fee.
- Application for Readmission - All formerly enrolled students who wish to come back to PBA must complete the application for readmission form. This is the first step in the re-admission process.
